What a Saturn sign is
It is the zodiac sign Saturn occupied at the moment you were born, which in plain terms is an angle: the zodiac is the ecliptic circle, the track Earth's orbit traces on the sky, divided into twelve arcs of 30 degrees, and Saturn is somewhere along it at every moment. A generation's marker more than a signature

The date is almost always enough

Retrograde is a different question
Saturn is retrograde on 36.4% of days — more than a third, roughly four and a third months of every year — so a retrograde Saturn at birth is ordinary rather than a portent. Retrograde is apparent backward motion seen from a moving Earth; it does not make the placement 'bad', and it is a separate question from which sign Saturn is in. Saturn can and does cross a sign boundary while retrograde, so on a changeover day the direction and the sign are simply two different facts. We print the direction as a secondary line beneath your result, because for the sign itself it decides nothing.
How we compute it
Saturn's position comes from the same ephemeris data professional astrology software relies on: an apparent geocentric longitude, which is why no birth city is needed. Signs are tropical, anchored to the March equinox rather than to the constellations, so a sidereal or Vedic calculator will usually show you the previous sign. Your day is the civil day in the timezone selected next to the time field, midnight to midnight. Without a birth time we compute for noon inside that day, and we also read Saturn at both of its ends: if the two ends sit in different signs — which for a planet this slow is very rare — you get both candidates instead of a verdict, and the exact instant of the change, found by narrowing the interval until it stops moving. With a birth time we compute for that instant instead.

Why does another calculator give me a different Saturn sign?
Usually because it is sidereal. A sidereal zodiac still uses twelve 30-degree signs, but anchors 0° Aries to the fixed stars and subtracts an ayanamsha — about 24 degrees on the Lahiri system in use today — which currently shifts most positions back by roughly one whole sign. We use the tropical zodiac, anchored to the equinox, which is what Western astrology means by a sign. The other, rarer reason is that you were born on one of the 0.23% of days Saturn changed sign and the other tool assumed an hour instead of showing both.
Was my Saturn retrograde, and does that change the sign?
Retrograde is a direction, not a sign: it tells you Saturn was moving backwards against the stars, a separate fact from which sign it was in.
